How to insert Text Fields, Check Boxes, Drop-Down Lists, etc. in a Word document to create a Form


Word has this nice feature which enables you to insert text fields, check boxes, drop-down lists, etc. into a document which you can use to create a Form.
These elements are available under the tab Developer in the Ribbon. You have to enable this tab if it is not already there in the Ribbon.
To enable the Developer tab, click the Office button Picture1. A drop-down list appears. Click the Word Options button from the bottom of the list. Word Options dialog box appears.
On the dialog box, under the category Popular (see on the left hand side), select option Developer tab in the Ribbon. Now, you can view the Developer tab in the Ribbon.
To insert a check box
1. Click the Developer tab.
2. Under the group Controls, click the Legacy Tools button.
3. From category Legacy Form, click the check box icon. Word creates a check box at the mouse cursor position.
If you wish, you can remove shading of the check box by clicking the Form Field Shading icon under Legacy Form.

The check box will not work until you enforce protection to the document.
To enforce protection
1. Under the Developer tab, within the group Protect, click icon Protect Document. A drop-down list appears.
2. From the drop-down menu, click Restrict Formatting and Editing. Restrict Formatting and Editing dialog box appears.
3. From this dialog box, select option Allow only this type of editing in the document option. A drop-down is enabled. Select Filling in forms option from the drop-down list. See below image.
4. Click the button Yes, Start Enforcing Protection. The Start Enforcing Protection dialog box appears, which prompts you to enter a password.
5. Enter a password and retype the same. Now, your document is protected and the check box becomes clickable.

Important: Remember that you cannot edit a protected document. To edit, you have to stop protection by entering a valid password. To stop protection, under the Developer tab, within the group Protect, click Protect Document. TheRestrict Formatting and Editing dialog box appears. From the bottom of the dialog box, click the Stop Protection button. Enter the valid password to remove protection.
To insert an ActiveX Control check box
1. Click the Developer tab.
2. Under the group Controls, click the Legacy Tools button.
3. From category ActiveX Controls, click the check box icon. A check box is created at the insertion point. See below image.
You can notice that the Design Mode icon is automatically enabled in theControls group. This is important to edit the check box properties.
1. Right-click the check box. From the right-click menu, click CheckBox Objectand then click Edit. The check is in edit mode. You can enter a name of the check box.
2. From the right-click menu, click Properties to control properties of the check box.
Note: The difference between a legacy check box and ActiveX control check box is that, ActiveX control check box provides advanced features which enables you to code in visual basic to make the check box behave in a certain way.
To insert a drop-down list
1. Under the Developer tab, within the group Controls, click the Legacy Tools icon. A drop-down list appears.
2. Click the Drop-Down Form Field icon from the drop-down list. A field is created at the cursor position.
3. Double-click the field. The dialog box Drop-Down Form Field Optionappears. Using this dialog box, we will enter items of the drop-down list.
4. Enter the first item in the field Drop-down item:. And, then click the Addbutton. The item is added to the list and you can view the item being added to the list “Items in the drop-down list”.
5. After entering all the items to the list, click OK.
Similarly, click the Text Field icon to insert a text field into your document. Double-click field to select the type of field (e.g. date field, number field, regular text, etc.), default text, date, etc., and various other properties.

    when i am using save as option the form was not working help me pls.............
